After downloading and completing this form, you may either:

The message MUST originate from a recognized Tufts or
Tufts Medical Center email address.

Print a copy and bring it to the Hirsh Health Sciences
Library (Sackler 4) with your Tufts or
Tufts Medical Center ID, or similar proof of status.

Tufts Medical Center ID, or similar proof of status.
Registration for others*
Tufts
Boston-campus alumni, retired faculty and retired staff should
register in person at the Library Service Desk Monday-Friday between
9am and 5pm. (Call 617.636.6706 for information.)
Visiting
Faculty, Scholars and Students should register in person at
the Library Service Desk Monday-Friday between 9am and 5pm and bring
a letter on letterhead from their sponsoring department stating
how long they will be staying and that the department would like
them to have library privileges.
Boston Library Consortium borrowers must bring a valid
BLC card and a current ID from their home institution and register
in person at the Library Service Desk.
*Important note to all library users: as a user of the Tufts University Hirsh Health Sciences
Library you accept responsibility for the return of all
materials borrowed from any of the Tufts University Libraries
or from other libraries through document delivery. All fees
incurred for lost, damaged or late items must be paid and
failure to pay such fees may result in a loss of library
privileges, an attachment of wages, or additional charges
on term bills. You also agree to abide by all HHSL policies
and you are responsible for copyright compliance as well
as Tufts University policies concerning the responsible
use of resources. |
